Each year, Region V+ AMHI distributes surveys for Region V+ consumers, mental health providers, and community partners to help evaluate the work we are doing and identify mental health gaps in Region V+.
Please take a moment to fill out the below survey that best fits your experience with mental health. Region V+ AMHI evaluates the results at the end of each year and considers the results in their planning efforts for the following year. We would not be able to do the work we do without your feedback.
If you have lived with or work with mental health experiences and leveraged resources in Region V+, please see one of these surveys.
